Permits to Work in Offices: 11 Things Systems Get Wrong

Permits to work are associated with refineries and heavy industry, which is why most office-based organisations assume the subject does not concern them.
Then somebody welds a bracket in a plant room, isolates a distribution board to install a socket, enters a riser to run cable, or works on a roof to service a chiller. Every one of those happens in ordinary commercial buildings, usually performed by a contractor, usually on a Friday, and usually with no permit at all.
A permit to work is simply a documented authorisation for a defined piece of high-risk work, issued by someone competent, valid for a stated period, and closed out on completion. Eleven things a system needs to get right, and the reason each one fails.
1. A permit is a control, not a form
The failure that undermines everything else.
The value lies in the conversation the permit forces: somebody competent examining the specific job, deciding what must be isolated, checking it has been, and authorising work under stated conditions. The paper is evidence of that assessment, not a substitute for it.
Systems fail when the form becomes the process. A permit signed in advance by someone who has not seen the job records an event that did not happen.
2. Know which work actually needs one
An office needs permits far less often than a plant does, and more often than nothing.
The realistic triggers in a commercial building are hot work involving naked flame, grinding or soldering; entry into confined or restricted spaces such as risers, tanks, ducts and some plant rooms; work on or near live electrical systems, or work requiring isolation; work at height on roofs and plant decks; and work affecting fire safety systems, where an alarm or suppression system is impaired.
That last category is routinely missed. Taking a detection zone out of service while cabling is done changes the building's fire strategy for the duration, and article 8 of the Regulatory Reform (Fire Safety) Order 2005 obliges the responsible person to take general fire precautions to ensure the safety of employees.
3. Confined spaces are a legal category, not a judgement call

The Confined Spaces Regulations 1997 apply where a place is substantially enclosed and there is a reasonably foreseeable specified risk. Risers, ducts, lift pits, tanks and some plant rooms can qualify, and the Regulations require that entry is avoided so far as reasonably practicable, and where it cannot be, that a safe system of work and emergency arrangements are in place.
An office that has never asked whether it has confined spaces has not established that it does not.
4. Isolation is the point, and it is usually the weak link
For electrical work, the permit exists to make isolation reliable.
Regulation 12 of the Electricity at Work Regulations 1989 addresses means of cutting off supply and isolation, and regulation 13 requires adequate precautions to prevent equipment made dead from becoming live while work is carried out.
The practical elements are lock-off devices under the control of the person doing the work, proving dead with a tested instrument, and warning notices. A permit that says "isolated" without specifying who isolated what, where the lock is, and who holds the key has documented an intention.
5. The issuer must be competent and must not be the one doing the work
The structural separation that makes the system meaningful.
Whoever issues the permit is authorising work on behalf of the organisation, which requires enough knowledge of the building and the hazard to know what conditions to impose. And they should not be the person performing the task, because the check disappears when the same individual assesses and executes.
In most offices this means a named facilities or engineering role, with named deputies, rather than whoever happens to be on site. The absence of a defined issuer is the most common structural gap in office permit systems.
6. It must say what the work actually is
Vague scope is where permits fail in practice.
"Electrical works, third floor" authorises almost anything. A permit should state the specific task, the exact location, the equipment involved, the hazards identified, the precautions required and the personnel authorised to carry it out. HSE's guidance on permit-to-work systems sets out what the document should establish.
The test is whether somebody arriving at the location could tell from the permit whether what is happening in front of them is what was authorised.
7. Time limits, and what happens when the job overruns
Permits are valid for a defined period, and jobs run late.
The failure mode is a permit that expires while work continues, or one issued open-ended so that expiry never arises. Both leave work proceeding without valid authorisation, and the second is worse because it looks compliant.
Build in a stated duration and an explicit route for extension, requiring the issuer to reassess rather than initial the original. A job that has overrun has usually changed.
8. Handback is a separate act
The step most often skipped, and the one that causes harm.
Handback is where the person doing the work confirms it is complete or suspended, the area is safe, isolations can be removed and systems restored. Without it, a system remains isolated when everyone believes it is live, or a fire detection zone stays disabled over a weekend.
Every permit needs a physical handback signature and a check that the area is clear before restoration. An impaired fire system restored on Monday because nobody signed off on Friday is a genuinely dangerous outcome.
9. Cancellation needs a route as well
Related and distinct. Work sometimes stops without finishing: conditions change, a hazard emerges, the contractor is called away.
The permit must be cancellable by the issuer at any point, and the conditions on cancellation stated. In practice this rarely happens because nobody has been told they can, so a permit system needs an explicit statement that any authorised person may stop the work.
10. In a leased building, there are two systems
The complication most tenants have not resolved.
The landlord or operator typically runs a permit system for the building fabric and services. You may be arranging work within your demise. Where those overlap, and they do at risers, ceiling voids, distribution boards and roof plant, two systems must interact or neither controls the risk.
Agree in writing which works require the building's permit, which require yours, and who authorises work in shared areas. Where more than one party has duties in respect of the same premises, the law requires co-operation and co-ordination, and this is one of the clearest cases.
11. Records are the audit trail
Permits are not disposable.
Retain them with the associated risk assessments and method statements, together with handback records. They evidence that the arrangement operated, and after an incident they are the first documents requested. Holding them alongside your other records across every site is where health and safety consultants and software are worth more together than either alone.

Rows four and eight are where harm actually occurs. Everything else determines whether the system works; those two determine what happens when it does not.
For international groups
Permit systems are one of the more transferable parts of a group safety framework, because the underlying logic is the same everywhere: authorise, isolate, control, hand back.
What varies is what the law requires around them. Several jurisdictions in this series prescribe who may perform particular categories of work, or require permits explicitly in national instruments rather than leaving them to the employer's system. Kenya's Act expressly addresses permits to work among its provisions, and jurisdictions with reserved specialist roles will expect the authorising person to hold local credentials.
The workable pattern is a group permit standard, with local confirmation of who may issue and what national rules add. Periodic health and safety audits test whether the system operated rather than whether it exists on paper.
